Admissions Registered Nurse

📋 Description

• Complete OASIS SOC visits for adult and geriatric home health patients.
• Perform nursing assessments and develop individualized care plans.
• Deliver nursing interventions, including med management and wound care.
• Monitor patient conditions and communicate changes to physicians.
• Educate patients and caregivers to promote safety and independence.
• Document assessments and services in electronic medical records.

🎯 Requirements

• Current New Jersey RN license in good standing.
• Minimum one year nursing experience, preferably in home health.
• OASIS and SOC experience preferred.
• Familiar with electronic medical records; HCHB a plus.
• Strong assessment, communication, organizational, and time mgmt skills.
• Ability to work independently delivering exceptional patient care.
