Admissions Registered Nurse

📋 Description

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients
• Perform thorough initial nursing assessments, including medication reconciliation, safety
• Develop individualized plans of care in collaboration with physicians, clients, caregivers, and
• Identify client needs and coordinate appropriate nursing, therapy, social work, and other home
• Review and execute physician orders and ensure the plan of care accurately reflects each client's
• Educate clients and caregivers regarding diagnoses, medications, treatments, safety, and the home

🎯 Requirements

• Current and unrestricted New Jersey Registered Nurse (RN) license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.
