Complete Start of Care assessments and develop care plans.
Conduct 1–3 SOC visits daily across Monmouth/Middlesex/Somerset/Union counties.
Perform skilled nursing tasks: med administration, wound care, treatments, and education.
Complete accurate OASIS documentation and SOC visits.
Educate patients and caregivers on disease management and home safety.
Collaborate with physicians and care team to plan care.

🎯 Requirements

Current RN license in New Jersey.
Minimum 1 year nursing experience; Home Health preferred.
OASIS documentation and SOC visits experience strongly preferred.
EMR familiarity; Homecare Homebase (HCHB) a plus.
Excellent assessment, communication, and time-management skills.
Passion for patient-centered home health care.
