Admissions Registered Nurse, Home Health

📋 Description

• Complete 1-3 SOC visits daily across Monmouth, Middlesex, Somerset, Union Counties
• Conduct comprehensive patient assessments and accurately complete OASIS documentation
• Develop individualized plans of care in collaboration with physicians and interdisciplinary care team
• Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education
• Educate patients and caregivers on disease management, medications, and home safety
• Monitor patient conditions and communicate changes appropriately

🎯 Requirements

• Current RN license in the State of New Jersey
• Minimum of one year nursing experience (Home Health experience preferred)
• Experience completing OASIS documentation and Start of Care visits strongly preferred
• Comfortable using EMR; Homecare Homebase (HCHB) experience is a plus
• Excellent assessment, communication, and time management skills
• Passion for providing exceptional, patient-centered home health care
