Admissions Registered Nurse, Home Health

📋 Description

• Complete 1–3 SOC visits daily across Monmouth, Middlesex, Somerset, Union.
• Assess patients and complete OASIS documentation.
• Develop care plans with physicians and the care team.
• Deliver skilled nursing: meds, wound care, treatments, education.
• Educate patients and caregivers on disease management and home safety.
• Monitor patient conditions and communicate changes.

🎯 Requirements

• Current RN license in New Jersey.
• At least 1 year nursing experience; Home Health preferred.
• OASIS and SOC experience preferred.
• Comfortable with EMR; Homecare Homebase (HCHB) a plus.
• Strong assessment, communication, and time management skills.
• Passion for patient-centered home health care.
