Admissions Registered Nurse, Home Health

📋 Description

• Provide rehabilitative care and skilled nursing for homebound clients.
• Travel to clients in designated geographic territories for home visits.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical/Client Services Manager.
• Follow up on, execute, and document doctors' orders.
• Conduct client assessments and coordinate case management.

🎯 Requirements

• NJ RN license.
• Minimum 1 year clinical (medical/surgical) experience.
• Graduation from accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills; EMR experience preferred.
• Ability to travel to cases as assigned.
