๐Ÿ“‹ Description

  • Provide comprehensive medication management services for patients with chronic and complex medical
  • Evaluate and optimize medication regimens in accordance with evidence-based guidelines and
  • Initiate, modify, monitor, and discontinue medications pursuant to applicable regulations and
  • Conduct patient assessments and medication reviews to identify drug therapy problems and
  • Monitor therapeutic effectiveness, adverse drug reactions, medication adherence, and patient safety
  • Collaborate with physicians and interdisciplinary teams to develop individualized treatment plans.

๐ŸŽฏ Requirements

  • Doctor of Pharmacy (PharmD) degree from an accredited college or school of pharmacy.
  • Current and unrestricted Pharmacist license in the applicable state.
  • Advanced Practice Pharmacist designation or eligibility in accordance with California regulations.
  • Completion of a Pharmacy Practice Residency (PGY1) or equivalent clinical experience. (Preferred)
  • Minimum of two (2) years of clinical pharmacy experience in ambulatory care, primary care, managed
  • Strong knowledge of chronic disease state management and evidence-based treatment guidelines.
