📋 Description

  • Conduct initial biopsychosocial intakes, including safety screening and behavioral health
  • Provide follow-up visits and asynchronous support using brief, evidence-based interventions
  • Build strong, trusted relationships with patients and engage them effectively through telehealth
  • Support patients in identifying barriers to care, increasing engagement, building self-management
  • Lead weekly interdisciplinary case review meetings with the Consulting Psychiatrist and relevant
  • Coordinate closely with Cerula Care’s behavioral health team, including the Consulting

🎯 Requirements

  • Must hold an active, unrestricted CO state license in counseling or social work
  • Master’s degree in Counseling, Clinical Mental Health Counseling, Social Work, Marriage and Family
  • At least three years of clinical care management experience, including experience working with
  • Experience conducting behavioral health intakes, psychosocial assessments, safety screens
  • Experience using evidence-based interventions such as Motivational Interviewing, Cognitive
  • Demonstrated ability to build strong relationships with both patients and healthcare providers.
