📋 Description

• Conduct scheduled telemedicine visits throughout the postpartum period to identify unmanaged
• Assess members experiencing Urgent Maternal Warning Signs or elevated blood pressure readings and
• Review blood pressure measurements and Edinburgh Postnatal Depression Scale (EPDS) results
• Serve as a clinical liaison between members and their obstetrics or primary care providers
• Support Maternity Navigators with evidence-based prenatal and postpartum education to promote
• Share insights from direct experience working with mothers and families to help inform and improve

🎯 Requirements

• Active Registered Nurse (RN) license in a compact state, with current eligibility in Tennessee or
• At least 5 years of experience providing prenatal and postpartum care in outpatient pediatrics
• Native or bilingual Spanish fluency across speaking, writing, and reading, with demonstrated
• Excellent interpersonal and communication skills, with a compassionate, empathetic, and culturally
• Strong organization and problem-solving abilities, with the flexibility to adapt effectively in a
• Confidence building trusted relationships with patients through virtual care and telemedicine.
