Care Coordinator, RN (MSHO)

📋 Description

• Coordinate face-to-face visits
• Manage the Elderly Waiver
• Conduct annual assessments (including PCA assessments)
• Review health needs, identify goals, and develop care plans
• Connect members with community and state resources
• Complete required documentation

🎯 Requirements

• Minnesota Registered Nurse license OR current MN Licensed Social Worker (LSW, LMSW, LISW, or LICSW)
• One or more years of experience with geriatric or disabled populations in case management preferred
• Valid driver's license
• Ability to work independently
• Access to a private and compliant home office space
• Creative problem-solving skills
