Care Manager (Denver, CO / Field-based)

📋 Description

• Schedule and perform quarterly supervisory home visits for compliance.
• Serve as field expert supporting members and caregivers with continuing care and appropriate use of
• Perform required home inspections to ensure safety and compliance.
• Partner with care team to monitor caregiver daily notes for training or support needs and document
• Maintain regular contact with caregivers (virtual or in person) based on service plan.
• Communicate concerns about member/caregiver health, safety, or unmet needs to RN or Case Manager.

🎯 Requirements

• Bachelor's degree or equivalent work experience.
• 2+ years bedside experience as CNA, QMAP, Direct Care Worker, Medical Assistant, etc.
• 1+ years call center, telephonic outreach, or customer service experience (required).
• Ability to exercise independent judgment and voice issues/trends.
• Strong verbal and written communication skills.
• Strong problem-solving and escalation skills.
