Care Manager Social Worker - Oncology Services

📋 Description

• Conduct comprehensive psychosocial assessments and develop individualized care plans addressing
• Identify barriers to care, including socioeconomic challenges, unmet social needs, impaired coping
• Coordinate care with physicians, nurses, medical providers, patients, families, community
• Provide counseling, education, patient advocacy, referrals, and navigation support to help patients
• Monitor, evaluate, and reassess care-management plans, adjusting interventions as patient
• Support transitions of care, discharge planning, risk stratification, and appropriate utilization

🎯 Requirements

• Bachelor’s or Master’s degree in Social Work from an accredited school of social work; a Master’s
• Current Maryland social work licensure, with the ability to maintain licensure and complete
• LCSW certification/licensure is required.
• Approximately 3–5 years of healthcare and/or care-management experience.
• Care-management certification, such as Accredited Case Manager (ACM), is preferred.
• CPR/BLS certification is required, unless exempt under an approved full-time remote arrangement.
