Case Manager, RN

📋 Description

• Conduct clinic nursing assessments to determine participant needs, preferences, and goals
• Actively participate in interdisciplinary team (IDT) meetings to develop participant care plans
• Deliver and document nursing interventions as agreed upon in participants' care plans
• Maintain a safe environment, promptly respond to physician orders, and correctly administer
• Collaborate with the WelbeHealth PACE Physician and other IDT members in coordinating participant
• Communicate with participants and their families to revise care plans

🎯 Requirements

• Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
• Unencumbered California RN License
• Current Basic Life Support (BLS)/First Aid Certification required
• Two (2) years of nursing experience in a clinical setting with a frail or elderly population
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification
• CPR and First Aid certification upon hire preferred
