Provide superior customer service and quality home care
Coordinate client services and emergent scheduling issues
Manage your caseload while proactively growing it
Build relationships with clients, referral sources, payors and community organizations
Develop strong, communicative relationships with the team
Monitor metrics (admissions, Medicare %, referrals by reason)

🎯 Requirements

Bachelor's degree preferred.
Medicare, Home Care Homebase, and OASIS experience a plus.
Prior supervisory experience a plus.
Track record of achieving goals.
Ambition to grow and advance.
Strong computer skills; EMR experience.
