Clinical Manager, Hospice

📋 Description

• Lead and support field staff (RNs, CNAs, aides) to ensure hospice care.
• Respond to and manage clinical calls from nurses in the field.
• Review referrals and hospital notes to support safe care planning.
• Lead IDG meetings and coordinate care with the interdisciplinary team.
• Oversee caseloads, adapting to admissions and discharges.
• Provide proactive leadership to support staff and daily needs.

🎯 Requirements

• Current RN license in good standing in Pennsylvania.
• Graduate of an accredited nursing program; BSN preferred where applicable.
• Hospice experience.
• Previous clinical management or leadership experience.
• Strong communication, interpersonal, and problem-solving skills.
• Ability to lead with initiative, adaptability, and sound clinical judgment.
