Clinical Manager, Hospice

📋 Description

• Lead and support field staff including RNs, CNAs, home health aides, and other interdisciplinary
• Respond to and help manage clinical calls from nurses in the field.
• Review referrals, hospital notes, and other clinical documentation to support safe and appropriate
• Lead IDG meetings and partner with the interdisciplinary team to coordinate care.
• Oversee and manage caseloads in a dynamic hospice environment.
• Provide proactive, flexible leadership to support staff and address day-to-day operational and

🎯 Requirements

• Current RN license in good standing in Pennsylvania.
• Graduate of an accredited and approved nursing program; BSN preferred where applicable.
• Hospice experience.
• Previous clinical management or other relevant leadership experience.
• Strong communication, interpersonal, and problem-solving skills.
• Ability to lead with initiative, adaptability, and sound clinical judgment.
