📋 Description

• Lead, inspire, and support field staff to deliver exceptional care
• Provide clinical oversight for Assistive Care State Programs clients throughout the service area
• Conduct client assessments and develop individualized care plans based on client needs and

🎯 Requirements

• Current Registered Nurse (RN) license in good standing
• Minimum of 2 years of RN experience
• Strong clinical assessment, organization, communication, and problem-solving skills
• Ability to provide clinical leadership and support in a community-based care environment
• Ability to manage client needs, documentation, state program requirements, and competing priorities
• Ability to travel throughout the service area for client visits, staff support, meetings, and
