Clinical RN Manager, Home Care

📋 Description

• Lead, mentor, and support a team of caregivers, including HHAs and CNAs, and other field staff
• Oversee client services, including onboarding, care plan development, service coordination, and
• Conduct client assessments, supervisory visits, service check-ins, and occasional field visits
• Build strong, trusting relationships with clients and families
• Respond to and resolve client, family, caregiver, and staffing concerns with professionalism and
• Partner with scheduling and office leadership to support staffing, coverage, and reliable service

🎯 Requirements

• Bachelor’s degree in healthcare, business, human services, or a related field with at least 2 years
• Experience in healthcare, home care, human services, case management, or service operations
• Prior leadership or supervisory experience in healthcare, human services, or a service-based
• Strong organizational, communication, relationship-building, and problem-solving skills
• Ability to manage competing priorities in a fast-paced office and field environment
• Comfort using computer systems for documentation, communication, scheduling, and reporting
