Field Nurse (Per Diem / Queens, NY)

📋 Description

• Conduct initial assessments of clients to determine care needs and develop individualized care
• Train and supervise non-skilled caregivers in providing personal care, companionship, and
• Monitor clients’ overall well-being and report any changes to the appropriate healthcare
• Educate clients and their families on effective care strategies, safety measures, and daily living
• Ensure compliance with agency policies, state regulations, and best practices in non-skilled care.
• Maintain accurate documentation of client care and progress, while ensuring confidentiality.

🎯 Requirements

• Bachelor's Degree (BSN) with current NYS License or compact License in good standing OR associate's
• Minimum 1–2 years of Nursing experience with concentration in homecare agency space.
• Experience in geriatric care or working with individuals with disabilities.
• Ability to travel within the 5 boroughs as needed.
• Current knowledge of state and federal home health regulations.
• Strong interpersonal and communication skills.
