Functional Wellness Supervisor

📋 Description

• Lead rehabilitation and functional wellness services at the Center to support independence, safety
• Provide oversight for Occupational Therapy, Physical Therapy, and integrated functional wellness
• Align clinical operations with PACE regulations, CMS requirements, state licensure standards, and
• Partner with the interdisciplinary team and center leadership to ensure clinically appropriate
• Manage staffing, scheduling, and workflows; coach performance and ensure reliable service coverage.
• Monitor key performance indicators (utilization, functional outcomes, and participant satisfaction)

🎯 Requirements

• Unrestricted Occupational or Physical Therapy license in the state of practice (or eligible for
• Two to four years of experience with at least one of those leading people including performance
• Demonstrated experience in geriatric rehabilitation, functional assessments, and evidence-based
• Strong knowledge of PACE regulations, CMS requirements, and state licensure standards.
• Experience developing or overseeing functional wellness programs, fall-prevention initiatives, and
• Ability to interpret and act on clinical and operational data, including functional outcome
