📋 Description

• Lead, supervise and mentor multidisciplinary care team members to deliver high value, empathetic
• Stay current with the latest research and advances in dementia care and translate this knowledge to
• Collaborate and provide necessary oversight of the interdisciplinary team by staffing questions
• Identify areas of improvements in the care model and technical platform so we can continue to

🎯 Requirements

• Board Certified in Internal Medicine (ABIM) or Family Medicine (ABFM).
• Subspecialty Certification in Geriatric Medicine from the ABIM or Certificate of Added
• 5+ years of clinical practice experience and an active medical license in at least one state
• Experience with and enjoy working in interdisciplinary care teams.
• Passionate about educating families and other clinical providers.
• Think systematically, breaking down complicated problems into manageable initiatives.
