Habilitation Manager

📋 Description

• Partner with Clinical Manager as internal case manager and advocate for clients.
• Participate in Person-Centered Planning meetings to support individualized goals.
• Collaborate with clients, families, case managers, and teams to implement and evaluate care plans.
• Conduct regular client visits to assess progress and ensure ISP compliance.
• Evaluate and document progress toward client goals and outcomes.
• Maintain ongoing communication with case managers, care coordinators, families, and stakeholders.

🎯 Requirements

• Bachelor's degree required
• Experience working with the I/DD population required
• Prior supervisory or leadership experience required
• Demonstrated ability to take on increasing responsibility and achieve results
• Strong organizational, communication, and relationship-building skills
• Ability to collaborate with clients, families, and interdisciplinary teams
