Habilitation Manager (I/DD services)

📋 Description

• Serve as case manager and advocate for Habilitation clients in Erie, PA.
• Participate in Person-Centered Planning processes.
• On-call rotation every 6 weeks (Mon-Mon).
• Collaborate with clients, families, and care teams to implement plans.
• Visit clients regularly to monitor progress and services.
• Supervise Habilitation Technicians per policy and state rules.

🎯 Requirements

• Bachelor degree is required.
• Must have 1 year experience with I/DD population.
• Prior supervisory experience a plus.
• Demonstrated record of taking on increased responsibility.
• Ambition to grow and advance beyond current position.
• Excellent communication and interpersonal skills.
