📋 Description

• Act as internal case manager and advocate for clients
• Participate in Person-Centered Planning processes
• Collaborate with clients, families, Case Managers, and teams
• Make regular client visits to evaluate progress
• Evaluate progress of goals
• Maintain communication with Case Managers/Care Coordinators

🎯 Requirements

• Bachelor's degree
• Minimum 1 year experience working with IDD population
• Prior supervisory experience (plus)
• Excellent communication and interpersonal skills
• Record of project success
