Screen for Health Home functional scale eligibility and conduct initial assessments
Collaborate with medical providers to develop and implement care plans
Provide direct service to caseload, coordinate patient services and care
Provide crisis intervention when needed
Conduct home visits and maintain documentation per Health Homes standards
Maintain patient confidentiality at all times

🎯 Requirements

1+ year of health homes, HARP or case management experience (required); Bachelor's degree
Experience navigating several data management systems, such as Salesforce
Bilingual Spanish strongly preferred
