Home Care Habilitation Manager

📋 Description

• Act as internal case manager and advocate for clients in the Habilitation program
• Participate in Person-Centered Planning processes
• Collaborate with client, family, Case Managers, Hab Techs to implement/evaluate ISP goals
• Make regular client visits to evaluate progress per ISP and service definitions
• Evaluate progress of goals in the ISP
• Maintain ongoing communication with Case Managers and Care Coordinators

🎯 Requirements

• Bachelor degree is required
• Must be a Qualified Professional - 1+ year post-degree I/DD experience
• Prior supervisory experience a plus
• Demonstrated record of taking on increased responsibility
• Ambition to grow and advance beyond current position
• Excellent communication and interpersonal skills
