Home Care Habilitation Manager

📋 Description

• Act as internal case manager and advocate for Habilitation clients.
• Participate in Person-Centered Planning processes.
• Collaborate with client, family, Case Managers, Hab Techs to implement goals in ISP.
• Make regular client visits to monitor progress and service delivery per ISP.
• Evaluate progress of goals identified in the ISP.
• Maintain ongoing communication with Case Managers and Care Coordinators.
• Supervise Habilitation Technicians per policy and state regulations.
• Participate in client orientations as directed by Clinical Manager.

🎯 Requirements

• Bachelor degree is required.
• Must be a Qualified Professional with 1+ year post-degree I/DD experience.
• Prior supervisory experience is a plus.
• Demonstrated record of taking on increased responsibility.
• Ambition to grow and advance beyond current position.
• Excellent communication and interpersonal skills.
