📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements: mobility, ROM, transfers, balance, strength.
• Implement wound prevention and repair with evidence-based dressings.
• Develop and revise individualized care plans: manual therapy, gait, balance.
• Document observations and interventions in EMR using a touchscreen tablet.

🎯 Requirements

• VA PT license.
• Graduate from an APTA-approved PT program (transcript or diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
