📋 Description

• Make home visits to clients in designated territories.
• Perform tests and measurements: mobility, ROM, strength, balance.
• Provide wound care and wound prevention using evidence-based dressings.
• Develop and adjust individualized care plans with manual therapy and gait training.
• Collaborate with nursing and other disciplines in the multidisciplinary team.
• Educate clients and families on rehab activities to promote independence.

🎯 Requirements

• Current Maryland Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduate from APTA-approved program or AMA CAAHEA accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
