📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements: mobility, ROM, balance.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop individualized care plans including manual therapy and gait training.
• Continuously assess and revise the PT plan; coordinate with nursing.
• Document observations and interventions using a touchscreen tablet.

🎯 Requirements

• Current MD Physical Therapist (PT) license.
• Graduation from an APTA/AMA-accredited program (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
