Make home visits to clients
Assess mobility, ROM, transfers, balance, strength
Implement wound care with evidence-based dressings
Develop and update PT care plans; include gait/balance training
Continually assess and revise PT care plan with nursing
Educate clients and document care using a touch-screen tablet

🎯 Requirements

MA Physical Therapist license
Minimum of one year of licensed PT experience
Ability to travel to assigned cases
Prior home health experience not required
