Making visits to clients in their homes
Perform diagnostics and measurements: mobility, ROM, balance, strength.
Develop and implement individualized care plans with manual therapy and gait training.
Continuously assess and revise the PT plan with nursing in the care team.
Educate clients and families in rehabilitative care to promote independence.
Accurately document observations, interventions and evaluations using a touch pad tablet.

🎯 Requirements

Current Massachusetts Physical Therapist license
New grads encouraged to apply; home health experience not required
Strong communication and collaboration skills
