Home Health Registered Nurse Preceptor

📋 Description

• Precept and mentor newly hired RNs and LPNs, providing one-on-one clinical training during home
• Monitor and manage progression of orientation, identify and address areas of opportunity.
• Provide 1:1 mentoring, support, and meaningful feedback — in person or via ZOOM — to new field
• Collaborate with the RN Clinical Manager and Clinical Educator to ensure ongoing staff competencies.
• Onboard new clinicians to help them build and manage their own caseloads.
• Case manage and admit new clients while precepting and mentoring clinicians.

🎯 Requirements

• Active RN license
• Prior Medicare-Certified Home Health experience — required
• A genuine passion for clinical education, coaching, and mentorship
• Strong clinical judgment and the confidence to lead by example
• Excellent communication, organization, and relationship-building skills
