Home Health Registered Nurse, RN

📋 Description

• Perform home health visits for adult and geriatric patients in Spartanburg and Greenville counties
• Follow designated care plans and administer medications, wound care, treatments, and procedures
• Monitor client conditions and report changes to Clinical/Client Services Manager
• Execute and document doctors’ orders and conduct client assessments as needed
• Case management and coordination with a focus on 1:1 care
• Utilize electronic medical records (EMR) and a touch pad tablet for documentation

🎯 Requirements

• Current South Carolina RN license
• Minimum of one year of clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to work independently and manage time effectively
• Strong interpersonal skills and attention to patient needs
• Solid computer skills; EMR experience preferred
