Home Health Speech Language Pathologist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, orders and goals.
• Revise care plans and participate in multidisciplinary care plan.
• Educate patients and families on rehabilitative care to promote independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Massachusetts SLP license required.
• At least 1 year of SLP experience.
• Prior home health experience preferred but not required.
