Home Health Speech Language Pathologist

📋 Description

• Make home visits to clients in designated territories.
• Perform evaluations to assess and treat speech, language, swallowing issues.
• Develop treatment plans per evaluation, orders, and goals.
• Assess and revise care plans with the multidisciplinary team.
• Educate patients and families in rehabilitative care.
• Document care using a touch pad tablet EMR.

🎯 Requirements

• A current MD license as Speech-Language Pathologist.
• ASHA CCC or equivalent credential.
• Education and work experience required for certification.
• Eligible to take ASHA exam for certification.
• Solid computer skills; EMR experience preferred.
