Home Health Visits - Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, transfers, balance, gait, strength, and safety.
• Implement wound care with evidence-based dressings.
• Develop and implement individualized PT plans (manual therapy, gait, balance).
• Educate clients and families in rehab care for safety and independent living.
• Document care in EMR using a touchpad tablet.

🎯 Requirements

• Current Physical Therapist license.
• Minimum 1 year of recent PT experience.
• Graduate from an APTA-approved program or AMA CAAHEA-accredited.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
