Home Health Visits - Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, strength, endurance, balance and safety assessments.
• Implement wound care using evidence-based dressings and topical agents.
• Develop and implement care plans with manual therapy, gait and balance training.
• Continuously assess and revise the PT care plan with nursing involvement.
• Educate clients and families on rehabilitative care for home living.
• Document observations, interventions and evaluations on a touch-pad tablet.

🎯 Requirements

• Current Physical Therapist (PT) license.
• Minimum one year of PT experience.
• Graduate from an APTA/AMA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
