Home Health Visits - Physical Therapist

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measures: mobility, ROM, transfers, stability, ambulation, strength, endurance, balance, safety.
• Utilize evidence-based dressings and topical agents for wound care.
• Develop and implement individualized care plans with PT interventions.
• Continuously assess and revise the care plan with nursing and the team.
• Educate clients and families on rehabilitative care and independence.
• Document observations and interventions on a tablet using EMR.

🎯 Requirements

• Current MD Physical Therapist license.
• Graduation from an APTA-approved program (transcript/diploma).
• One year of clinical PT experience required.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
