Follow a designated care plan per patient needs
Make home visits to clients in Back Bay, Beacon Hill, Fenway, South End and nearby areas
Administer medications, wound care, treatments, and procedures
Monitor client conditions and report changes to Clinical or Client Services Manager
Document observations and interventions on a touch pad tablet
Perform client assessments and coordinate case management

🎯 Requirements

Massachusetts RN license (current)
Graduation from an accredited nursing program
Prior RN experience in sub-acute or acute care
