📋 Description

• Conduct head-to-toe assessments and spot subtle patient changes.
• Deliver acute and post-acute care (CHF, COPD, wound care, IV therapy).
• Triage and independently manage urgent clinical situations at home.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care, and documentation.
• Work as part of a multidisciplinary team to help clients stay at home.

🎯 Requirements

• MA RN license in good standing.
• Graduation from an accredited nursing program with transcript or diploma.
• At least 1 year of hands-on adult/geriatric care in listed settings.
