Home Health Visits Registered Nurse

📋 Description

• Perform head-to-toe assessments to identify changes in patient condition
• Provide skilled nursing care for acute and post-acute conditions (CHF, COPD, wound care, IV therapy)
• Triage and manage urgent clinical situations independently in home settings
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

🎯 Requirements

• Current nursing license in good standing in Massachusetts
• Graduation from an accredited nursing program
• At least one year of recent hands-on medical experience with adult/geriatric population
• Independent work ability with clinical decision-making
• Strong problem-solving skills in unpredictable home environments
• Efficient with point-of-care documentation and technology
