📋 Description

• Use strong head-to-toe assessment skills
• Identify subtle changes in patient condition
• Utilize acute and post-acute care experience (e.g., CHF, COPD, wound care, IV therapy
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation

🎯 Requirements

• Current nursing license in good standing in the state of Massachusetts
• Graduation from an accredited and approved nursing program, as indicated by school transcript or
• At least one year of current or recent hands-on medical experience within the past two years with
