Making home visits to clients in designated geographic territories.
Administering medications, wound care, treatments, and comfort care.
Monitoring clients' conditions and reporting changes to the Clinical or Client Services Manager.
Following up on doctors' orders and documenting appropriately.
Performing client assessments as needed.
Occasional on-call after hours and weekends.

🎯 Requirements

A current VT and NH RN license.
A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
Prior home care and/or hospice experience strongly preferred, but not required.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.
