📋 Description

• Develop, manage, and maintain strong relationships with assigned referral partners, including
• Serve as the primary point of contact for referral sources, ensuring consistent communication and
• Collaborate closely with discharge planners, case managers, social workers, and clinical teams to

🎯 Requirements

• Experience in hospice, home health, or healthcare account management strongly preferred.
• Solid understanding of hospice philosophy, eligibility criteria, and interdisciplinary care.
• Minimum of two (2) years of experience working with physicians, hospitals, insurance providers
• Strong communication, presentation, and relationship-building skills.
• Excellent organizational skills with the ability to manage multiple accounts and priorities.
• Ability to work independently and collaboratively within a multidisciplinary team.
