📋 Description

• Internal case manager and advocate for clients in habilitation services.
• Lead Person-Centered Planning and develop individualized goals.
• Collaborate with clients, families, Case Managers, Habilitation Techs, and teams.
• Visit clients regularly to assess progress and ISP alignment.
• Monitor progress toward goals and outcomes.
• Maintain strong communication with stakeholders.

🎯 Requirements

• Bachelor's degree required.
• Experience with I/DD population required.
• Prior supervisory or leadership experience required.
• Ability to take on responsibility and achieve goals.
• Strong communication, organization, and relationship-building.
• Ability to collaborate with clients, families, and teams.
