Intake Coordinator

📋 Description

• Coordinates the end-to-end intake process for prospective participants by partnering with
• Coordinates home visits, center tours, assessments, and follow-up appointments to ensure timely
• Maintains proactive communication with the Outreach & Enrollment, and clinical pre-enrollment
• Partners with clinical teams to identify, coordinate, and facilitate additional assessments
• Delivers a welcoming, participant-centered experience by guiding prospective participants and
• Maintains accurate participant information, referral activity, and intake documentation within CRM

🎯 Requirements

• Bachelor's degree in Healthcare Administration, Business Administration, Marketing, or a related
• Minimum of one year of experience in healthcare, enrollment, participant services, care
• Demonstrated ability to coordinate multiple priorities with strong attention to detail and timely
• Experience working with Medicare, Medicaid, state and county eligibility programs preferred.
• Strong communication and collaboration skills.
• Demonstrated ability to work independently while collaborating across teams.
