Make home hospice recertification visits to clients in designated territories.
Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
Follow up on, execute, and document doctors' orders.
Perform client assessments as needed.

🎯 Requirements

A current PA NP license.
At least 1 year of clinical experience (medical/surgical).
Home care, hospice or palliative care experience preferred but not required.
Strong computer skills; EMR experience preferred.
Ability to travel to assigned cases.
Strong interpersonal skills.
