Lead Care Professional

📋 Description

• Provide compassionate, professional care to older adults in their homes
• Assist with personal care, mobility, transfers, companionship, meals, and daily routines
• Help with new client starts to create a smooth and positive care experience and assist with
• Review care plans and visit notes to understand each client’s needs and preferences
• Provide hands-on support, guidance, and encouragement to other caregivers
• Support clients with dementia care and end-of-life care needs

🎯 Requirements

• 4+ years years of professional caregiving experience (active CNA, HHA, LPN, or RN credential is
• Open weekday availability and the ability to rotate weekend on-call shifts
• Reliable transportation and willingness to travel up to 60 minutes to client homes
• Comfortable working in different home environments: houses, apartments, buildings with stairs or
• Ability to accept shifts on short notice when needed to help cover gaps
• Ability to meet physical requirements of the role, including assisting with mobility, transfers
