Licensed Practical Nurse

📋 Description

• Provide skilled nursing visits to clients in designated territories.
• Administer medications, wound care, and physician-ordered treatments.
• Monitor client conditions and report changes to the care managers.
• Complete comprehensive nursing assessments as needed.
• Execute and document physician orders accurately and timely.
• Coordinate care with physicians and the interdisciplinary team.
• Document client assessments and outcomes in BAYADA EMR.

🎯 Requirements

• Current New Jersey LPN license.
• At least 1 year LPN clinical experience.
• Graduate of an accredited nursing program.
• Home health experience preferred.
• Ability to work independently and manage time.
• EMR experience preferred.
